Before you begin your application you will need to ensure you have the fowlling ready to upload, as you will not be able to submit the form with the details.

• You have read an understood the responsibilities of a Voluntary Licenced Chaperone
• You have the contact details for 2 references
• You have a head and shoulders image of yourself for use on your licence
• You have apply for an Enhanced DBS check - Apply for an Enhanced DBS check
• You have completed your online eLearing safeguarding course, and have the PDF certificate to upload - Complete your online eLearing safeguarding course
• You have a digital copy of your First Aid certificate to upload (if you have one)
